Florida Statutes, Rule 6C-6.001 FAC, and Rule 6C-6.007 FAC outline the immunization requirements for students attending state colleges and universities, including 91°µÍø. To ensure compliance, incoming students must complete the 91°µÍø Health History and Immunization form and provide the necessary documentation.
Vaccinations offer a highly desirable and effective means of preventing many extremely
valuable diseases. Student Health Services provides a wide range of immunizations
to enrolled 91°µÍø students, including:
→ MMR (Measles, Mumps, Rubella)
→ Hepatitis B
→ Human papilloma virus (HPV)
→ Tetanus
→ Tetanus Diptheria [Td]
Consult with your health care provider or the 91°µÍø Student Health Services for more
information about these vaccines.
I. 91°µÍø Health History and Immunization Form:
All incoming students are obligated to complete this form, and it should be submitted
directly to Student Health Services (not to Admissions or other departments).
Your form must be accepted before you can register for classes.

II. MMR (Measles, Mumps, Rubella):
1. Provide proof of two (2) MMR shots, received at least 28 days apart.
- These vaccinations must have been administered after your first birthday.
- Vaccinations must have been received in 1968 or later
2. OR Provide proof of immunity to Rubella, Rubeola, and mumps through lab results.
III. Hepatitis B and Meningitis:
1. Provide proof of Hepatitis B immunization or sign a declination. AND
2. Provide proof of Meningitis immunization or sign a declination.
Other immunizations recommended by the American College Health Association (ACHA)
and the Advisory Committee on Immunization Practices (ACIP) include:
→ Varicella (chickenpox)
→ Polio
→ Tetanus,
→ Diptheria,
→ Pertussis (Td or Tdap)
→ Human papillomavirus (HPV)
→ Influenza
→ Hepatitis A
→ Pneumonia
1) Students born prior to January 1, 1957 are exempt from MMR requirement;
2) Religious exemption must be accompanied by a signed statement from your church
or a personal statement of your religious tenant and student/guardian completion of
vaccine waiver; and
3) Medical exemption requires specific, written documentation on office letterhead
signed by a physician. The student/guardian must also complete the 91°µÍø vaccine waiver.
Temporary deferments will be allowed if condition is documented on office letterhead
and signed by a physician, for pregnancy/possibility of pregnancy, breastfeeding or
illness.
